Please Note: Always Hope Behavioral Health Services is not currently participating with Blue Cross Blue Shield of Mississippi. Patients with BCBSMS are welcome to receive services through our self-pay options.

Louisiana Residents

Insurance-covered services may be available depending on your insurance plan and eligibility.

Please submit a Request for Services form, and our team will verify your insurance benefits and contact you regarding available scheduling options.

Self-pay services are also available.

Self-pay options are available for patients who prefer not to use insurance or whose insurance is not currently accepted by the practice.

Current self-pay rates include:

• Initial Psychiatric Evaluation: $250

• Follow-Up Medication Management: $150

Additional Information:

• Specialized evaluations, forms, letters, and documentation services may incur additional fees.

• Patients may request a superbill for services received. Reimbursement is not guaranteed and depends on the patient's insurance plan and out-of-network benefits.

Payment Policy:

•Patients are asked to maintain a valid card on file through the patient portal for the processing of copays, deductibles, coinsurance, self-pay fees, and applicable appointment fees.

•Copays, deductibles, coinsurance, and self-pay fees are due before the scheduled appointment time.

• Appointments may be canceled if required payment has not been received prior to the appointment.

• Insurance benefits are verified as a courtesy and are not a guarantee of payment.

• Patients are responsible for services not covered by their insurance plan.

• Patients may request a superbill for services received. Reimbursement depends on the patient's insurance plan and out-of-network benefits.

• Fees may apply for forms, letters, and documentation requests.
