Payment is due at the time of service for applicable copays, deductibles, coinsurance, and self-pay fees.

Self-pay options are available for patients who prefer not to use insurance or whose insurance is not currently accepted.

Current Self-Pay Rates :

• Initial Psychiatric Evaluation — $200

• Medication Management Follow-Up — $125

• Extended Medication Management Follow-Up — $150

• ADHD Evaluation — $265

• Medication Management Membership Plan— $45/month

Cancellation & No-Show Policy

• No-show fee — $100

• Late cancellation or rescheduling fee — $75

Additional Information

• FMLA, disability, workplace accommodation, and other specialized forms or letters may require an additional fee.

• Documentation requests requiring an appointment or additional clinical assessment may require a separate visit.

• Patients may request a superbill for eligible services. Reimbursement is not guaranteed and depends on the patient’s insurance plan and out-of-network benefits.
